About the role

The LVN provides compassionate end-of-life care by implementing established plans of care and delivering nursing treatments. They are responsible for monitoring patient conditions and collaborating with an interdisciplinary hospice team to ensure positive outcomes.

This is a PRN position needed in the following areas

Ft. Worth/HEB/Grapevine/Colleyville/Mansfield and Denton

The Licensed Practical/Vocational Nurse (LPN/LVN) provides compassionate, high-quality patient care under the direction and supervision of a Registered Nurse (RN). This role is responsible for implementing the established plan of care, delivering nursing treatments, monitoring patient conditions, and collaborating with the interdisciplinary hospice team to promote positive patient outcomes and quality end-of-life care.

Essential Responsibilities

Provides direct patient care as defined in State Nurse Practice Act.

Implements current nursing practice following a comprehensive assessment and the plan of care.

Provides accurate and timely documentation of patient services to reflect the plan of care.

Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.

Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group. Examples include hospice aide, MSW, and Clinical Supervisor.

Uses equipment and supplies effectively and efficiently.

Provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications.

Participates in personal, professional growth and development. Also participates in quality assessment performance improvement teams and activities.

Performs other duties as assigned by the registered nurse.

Qualifications

  • & Skills
  • Current and unrestricted Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license as required by state regulations.
  • Knowledge of hospice and palliative care principles preferred.
  • Strong clinical assessment, communication, and documentation skills.
  • Ability to work independently and collaboratively within an interdisciplinary team.
  • Commitment to delivering compassionate, patient-centered care.
  • Physical Requirements
  • Ability to travel to patient residences and care facilities as required.
  • Ability to lift, move, and assist patients in accordance with organizational safety guidelines.
  • Ability to perform job duties that require standing, walking, bending, and reaching for extended periods.
  • Must be a licensed driver with an automobile that is insured in accordance with state requirements and in good working order.
  • Possesses and maintains current CPR certification.

Requirements

Candidates must hold a current and unrestricted LPN or LVN license and a valid CPR certification. A valid driver's license, insured vehicle, and the ability to travel to patient residences are required.
