What You'll Do As a Case Manager, you'll play a key role in helping clients successfully navigate treatment and transition into recovery by: Providing comprehensive case management services for clients with moderate to high clinical and psychosocial needs.
Coordinating discharge planning and transition of care to ensure continuity of services.
Facilitating interdisciplinary case reviews and communicating client progress, barriers, and next steps.
Assisting clients with accessing housing, employment, transportation, insurance, benefits, community resources, and recovery supports.
Building collaborative relationships with referral sources, community agencies, courts, probation officers, and other external partners.
Advocating for clients while promoting accountability and independence.
Maintaining timely, accurate, and compliant documentation within the electronic health record (EHR).
Participating in quality improvement, compliance initiatives, and team meetings.
Supporting a positive, recovery-focused treatment environment.
Performing other duties as assigned.
What We're Looking For Required Qualifications Three (3) years of behavioral health experience with demonstrated case management competencies. Experience documenting within an Electronic Health Record (EHR). Current CPR/First Aid certification or ability to obtain within 30 days of hire.
Bachelor's degree in Social Work, Psychology, Counseling, or a related field. Two (2)+ years of experience in residential or outpatient substance use disorder or co-occurring treatment. Training in Motivational Interviewing and/or Trauma-Informed Care. CADAC or equivalent certification, as applicable under Iowa requirements.
