  • Mt Lebanon Blvd - Pittsburgh, PA 15234Chartiers Center is a private, non-profit corporation funded by the state, county, 3rd party insurances and private funds. Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization, community outreach, day, and recreational services for adults with intellectual disabilities, homeless outreach and housing program. As a Housing/Re-entry Coordinator with Chartiers Center you are responsible for locating and identifying housing opportunities, providing referrals, linkages and support related to housing, benefits, and mental health/physical health care to individuals and their family experiencing homelessness who also have mental health issues and justice system involvement.

  • Duties include: Collaboration with inpatient unit/emergency departments, jails and legal system to facilitate coordination of care is a primary function of this position. Serve as a monitor and clinical bridge to implement necessary strategies and support the overall wellness of the individuals served/identified, particularly as it relates to the person/family’s ability to secure and retain permanent housing.   • Qualifications: Bachelor’s degree in Social Work, Psychology, Counseling, Nursing or related field and at least one year experience in justice related services and experience working with chronically homeless/assisting homeless individuals and families to gain permanent housing. OR Associate's degree Social Work, Psychology, Counseling, or related field and two years' experience in justice related services and experience working with chronically homeless/assisting homeless individuals and families to gain permanent housing. Requirements

Requires a Bachelor's degree with one year of experience or an Associate's degree with two years of experience in justice-related services. Must have a background in assisting chronically homeless individuals and families in gaining permanent housing.
