Position Highlights: Part Time (1 day per week)
Coordinates patient care for assigned patients utilizing the nursing process.
What you will do
Utilizes the nursing process of assessing, planning, implementing, and evaluating all nursing care provided within the Cardiac and Pulmonary rehab settings.
Demonstrates knowledge of exercise principles and development of an appropriate exercise/rehab plan based on cardiac and pulmonary rehab patient evaluation and assessment. Performs ongoing evaluations of patient's compliance and response to therapy, and adjusts plan of care accordingly.
Initiates, provides, or delegates patient/family teaching at a level of expertise and documents appropriately, noting the recipients’ participation and response including pre-procedure calling and education.
Acts as a patient/family advocate and intervenes if necessary to ensure that patient care is not compromised.
Participates in planning, scheduling, and coordinating patient care.

Requires an active Registered Nurse (RN) license in the state of practice. Prefers 1-3 years of experience in a clinical or hospital environment.
