The Case Manager addresses patients' socioeconomic needs and coordinates care linkages between HIV testing sites and the medical clinic. They are also responsible for conducting community outreach and presentations regarding HIV, STIs, and reproductive health.
The HIV Case Worker (CW) position under the general administrative direction of the Director, addresses the patient’s socioeconomic needs and assists the rest of Medical Care Coordination (MCC) team with patient monitoring, following up service linkages, assisting the team in updating care plan results, following-up with patients and tracking outcomes. Additionally, the Case Worker should act as the liaison between HIV Counseling and Testing sites and the medical clinic to ensure that new patients are enrolled in medical care seamlessly and in a timely fashion.
