Family Health Care is currently seeking qualified candidates for our Medicare Annual Wellness Visit LPN position!
The Medicare Annual Wellness Visit (AWV) LPN supports Baldwin Family Health Care’s (BFHC) Medicare population by coordinating and completing Medicare Annual Wellness Visits (AWVs) at assigned clinic sites. The LPN conducts scheduled face-to-face individual Medicare Annual Wellness Visits in the clinic setting and, when approved by the Chief Medical Officer, via telephone. The LPN completes all required documentation, assessments, and clinical workflows in accordance with established BFHC policies, procedures, protocols, standing orders, and CMS guidelines. Performs all responsibilities according to best practices in the field and reflecting the mission and vision of Baldwin Family Health Care.
