Practice for this role

About the role

Provide in-home support services and individualized care planning for families with children at risk of removal. Coordinate with treatment teams, schools, and courts while maintaining detailed documentation in electronic record systems.

SAFY is seeking a full-time Case Manager I for our High Fidelity Wraparound program in Louisville, Kentucky. As a Case Manager, you will be responsible for providing in-home support services for families whose children are at risk of removal. This position will be based in Louisville but will be mainly working in the field. The State service region is Jefferson County.

High Fidelity Wraparound (Wraparound) is an evidence-based approach rated as “Promising” on the Title IV-E Prevention Clearinghouse. Wraparound centers on individualized care planning and coordination to support children and families with complex needs. The core principles of Wraparound include family voice and choice, natural supports, team-based, collaboration, community-based services, individualized planning, strength-based approaches, persistence, outcome-based, and cultural competence. Essential to the success of Wraparound programming is ensuring staff are adequately trained, the program is sustainable, and there are adequate resources to support the needs of youth and families enrolled in the Wraparound program.

The proposed program aims to achieve the following outcomes for children and families:

Prevention of Foster Care Involvement: By providing targeted support, Wraparound intends to decrease the need for out-of-home placements and maintain the stability of families.

Improved Family Functioning: Wraparound will help families develop stronger communication skills, conflict resolution techniques, and coping mechanisms to navigate challenges more effectively.

Enhanced Youth Well-being: At-risk youth will experience improved emotional well-being, academic performance, and overall life satisfaction through the program's holistic approach.

Strengthened Support Systems: Families will establish a robust network of community resources, professionals, and social connections to provide ongoing support beyond the program.

Who We Are

At SAFY, our mission is Preserving Families & Securing Futures. For 40 years, our work has rested on a single, radical idea – children belong in families. We believe that every child and family deserve the opportunity to reach their full potential. We are a team of 500 employees across seven states providing services such as Family Preservation & Reunification, Behavioral Health, Therapeutic Foster Care, Adoption Services and Older Youth Services so the families and children we serve can build resilience and thrive.

What You’ll Do

Work independently and create your own schedule based on your caseload and availability of your assigned families.

Provide in-home, direct services to families and children in your caseload. Generally, caseloads consist of 10-12 families that you’ll see weekly.

Create documentation for all services you provide in our Electronic Record System.

Participate in rotating on-call schedule with the other team members. Generally, we're on call 1 week every 2 months.

Develop and maintain consultative and liaison relationships with DCBS and other members of the youth’s treatment team (biological parents, school, etc.)

Collaborate with outside parties including family supports and relatives, schools, courts, hospitals, doctors, etc.

Travel to families’ homes in and around Louisville – mileage reimbursement is included. Overall hours being worked outside of the office is estimated to be around 60%, which includes traveling to and from visits and time spent at the home conducting a home visit.

Set your schedule in collaboration with the team. Generally, we work standard hours of around 9 am – 6pm, however there are times when you may need to work until 8pm or later, based on the needs of families on your caseload.

What You Bring

We are looking for people from diverse backgrounds and experiences who are inspired by our noble mission to make a difference in the lives of the children and families in our care. You’ll be right at home if you are willing to learn from successes as well as setbacks, persevere in challenges and adversity, cultivate collaborative relationships, understand and leverage diversity, advocate for equity, aspire to work inclusively, and partner to drive your work and that of your team to the next level.
