Home Health & Hospice RN CM Signature Healthcare at Home, part of The Pennant Group, is seeking a full‑time Registered Nurse Case Manager to serve patients along the Oregon Coast. Position Summary

The RN Case Manager provides skilled nursing care, comprehensive case management, and compassionate support to home health and hospice patients. You will coordinate interdisciplinary services, ensure high‑quality outcomes, and build meaningful relationships with patients and families in their homes.

Key responsibilities

Clinical Assessment — Perform initial and ongoing patient evaluations, develop individualized care plans, and deliver evidence‑based nursing interventions.

Care Coordination — Collaborate with physicians, therapists, social workers, and caregivers to ensure seamless, patient‑centered care.

Case Management — Manage a balanced caseload, monitor progress, adjust care plans, and advocate for patient needs.

Hospice Support — Provide end‑of‑life care grounded in dignity, comfort, and family guidance.

Documentation — Maintain accurate, timely clinical records in accordance with regulatory and organizational standards.

Patient Education — Teach patients and families about disease processes, medications, symptom management, and safety

Qualifications

  • Minimum one year of clinical nursing experience (home health or hospice preferred)

  • Ability to work independently in a field‑based environment
  • Valid driver’s license and reliable transportation
