Physical Therapist All Heart Home Care | Nashville Area | FT All Heart Home Care is seeking a compassionate and skilled Physical Therapist (FT) to join our home health team in the Murfreesboro area. This role offers flexible, as-needed scheduling while providing personalized, one-on-one care to patients in the comfort of their homes. Key Responsibilities: Perform in-home evaluations to assess patient needs and develop individualized treatment plans Provide skilled physical therapy services to improve mobility, strength, balance, and overall function Monitor patient progress and adjust treatment plans as appropriate Educate patients and caregivers on home exercise programs, safety, and injury prevention Complete accurate and timely clinical documentation in compliance with company policies and state regulations Coordinate care with physicians, nurses, case managers, and other members of the healthcare team Travel to patient homes within assigned service areas
