Physical Therapist (PT) | Full-Time & Part-Time All Heart Home Care | Columbia, TN All Heart Home Care is seeking compassionate and skilled Physical Therapists (Full-Time and Part-Time) to join our growing home health team in the Columbia, Tennessee area. Key Responsibilities Perform in-home evaluations to assess patient needs and develop individualized treatment plans Provide skilled physical therapy services to improve mobility, strength, balance, and overall function Monitor patient progress and modify treatment plans as appropriate Educate patients and caregivers on home exercise programs, safety measures, and injury prevention Complete accurate and timely clinical documentation in accordance with company policies and state regulations Collaborate with physicians, nurses, case managers, and other healthcare professionals to ensure coordinated patient care Travel to patient homes within the assigned service area
