The Physical Therapist is responsible for providing comprehensive therapy services to residents, including evaluations and the creation of individual treatment plans. They manage the full continuum of care from inpatient to outpatient services while focusing on holistic prevention.

In this role, you will be responsible for providing physical therapy services to residents, including evaluations, planning individual treatment programs and goals, actual treatments, discharge planning, and performing other related duties as required.

Requirements

The ideal candidate will meet the following requirements: Bachelor’s degree in Physical Therapy Valid Physical Therapist license or eligibility for license in the state(s) where services are rendered Experience in a skilled nursing and/or outpatient rehab environment (preferred) Team members are eligible for a generous benefit package including health benefits (medical, prescription, dental and vision), flexible spending accounts, life insurance, disability programs, 401(k) plan (with 4% company match after one year of employment), paid time off and holidays, and much more!
