The Licensed Practical Nurse (LPN) in a Skilled Nursing Facility (SNF) plays a critical role in delivering high-quality nursing care to residents with diverse and complex health needs.

This position involves closely monitoring patients' conditions, administering medications, and collaborating with interdisciplinary teams to develop and implement individualized care plans.

The LPN-SNF ensures compliance with healthcare regulations and facility policies while maintaining accurate and detailed patient records.

By providing compassionate and attentive care, the LPN supports residents' physical and emotional well-being, promoting recovery and quality of life.

Ultimately, this role is essential in fostering a safe, supportive, and therapeutic environment within the skilled nursing setting.

Minimum Qualifications

  • Current and valid Licensed Practical Nurse (LPN) license in the state of employment.
  • Completion of an accredited practical nursing program.
  • Basic Life Support (BLS) certification.
  • Minimum of one year experience in a skilled nursing or long-term care setting preferred.
  • Strong understanding of nursing principles, medication administration, and patient care documentation.

Preferred Qualifications

  • Experience working in a Skilled Nursing Facility (SNF) or long-term care environment.
  • Certification in geriatric nursing or wound care.
  • Familiarity with electronic health record (EHR) systems.
  • Additional certifications such as IV therapy or medication aide certification.
  • Demonstrated ability to work effectively in interdisciplinary teams.

Responsibilities

  • Administer prescribed medications and treatments accurately and timely according to established protocols.
  • Monitor and document patients’ vital signs, symptoms, and overall health status, reporting any changes to the supervising Registered Nurse or physician.
  • Assist with activities of daily living (ADLs) such as bathing, dressing, and mobility support to enhance patient comfort and independence.
  • Collaborate with healthcare team members including physicians, therapists, and social workers to coordinate comprehensive care plans.
  • Maintain compliance with federal, state, and facility regulations, including infection control and patient safety standards.
  • Educate patients and their families on health management, medication usage, and post-discharge care instructions.
  • Participate in quality improvement initiatives and staff training to enhance patient care outcomes.
