About the role

Coordinate individualized treatment plans and promote patient self-management to improve overall wellness and independence. Collaborate with physicians and care teams to assess patient progress and identify barriers to meeting healthcare goals.

Overview Work with the current staff to complete the care team in hopes to better improve patient care and the patient's overall experience. This is a part-time position that will report in clinic 2 to 3 times a week reporting to Dr. Mary Massary.

Responsibilities

Promotes patient self-management and empowers patient/ families to achieve maximum levels of wellness and independence Responsible for working with patient and patient's care team to coordinate change readiness, needs assessment and develop and individualized treatment care plan Collaborates with the patients, physician, and other care teams’ members in assessing the patient's progress toward individual health care goals Assesses barriers when patient has not met treatment/ goals, is not following treatment plan of care, or has not kept important appointments Perform other clinical nurse duties as needed

Qualifications

  • Graduation certificate from accredited LPN / RN School of Nursing 2 years related experience and/or training LPN or RN active license Prior Family Medicine experience preferred, but not required About MANA Family Medicine MANA Family Medicine Clinics provide comprehensive healthcare for every member of the family at all stages of life.
