In this role, you will independently perform home visits to conduct functional, psychosocial and environmental safety assessments, determine eligibility of enrollees, and establish an individual plan of care that enables them to remain safely at home despite long-term care needs. Multiple openings in Dutchess County as a primary location. We also have Ulster, Orange Rockland, Sullivan and Putnam. MJHS rewards full time Assessment Nurses with a higher reimbursement rate when conducting extra visits.
Current NYS RN license and registration Associates degree required; Bachelor of Nursing degree preferred Bilingual BSN new graduates encouraged to apply One year of experience in Homecare, acute care or community health nursing preferred. Working knowledge of Medicare, Medicaid and NYS DOH standards and regulations Bilingual preferred Min USD $90,000.00/Yr. Max USD $110,000.00/Yr.
Candidates must have a current NYS RN license and an associate degree, with a preference for a Bachelor of Nursing degree. Bilingual BSN new graduates are encouraged to apply, and one year of relevant nursing experience is preferred.
