At MJHS, our dedicated staff provides a full range of advanced clinical home care services, including patient assessment, medication management and patient/family education. In this role, you will evaluate the home care needs of our patients. You will also review patient insurance information, coordinate activities involved in each individual care plan and make sure that pertinent findings are shared with family and professional caregivers. Your excellent communication, documentation and time management capabilities will be essential to your success. Candidates must hold a current NYS RN license and registration with at least one year of acute medical-surgical nursing experience. A Bachelor's degree in nursing and CHHA experience are preferred.
