About the role

The Admission Nurse is responsible for initiating high-quality home health services by conducting comprehensive patient assessments and creating individualized plans of care. They ensure regulatory compliance and provide essential education to patients and caregivers to facilitate a smooth transition into home care.

The position will cover Queens with coverage for Brooklyn when needed.

This role conducts comprehensive home assessments to determine skilled care needs, establishes an individualized plan of care in collaboration with the interdisciplinary team, and ensures timely, accurate documentation that meets regulatory, payer, and agency requirements.

The Admission Nurse provides patient and caregiver education, evaluates home safety, and confirms eligibility for services.

Through strong clinical judgment, excellent communication skills, and a patient‑centered approach, the Admission Nurse helps ensure a smooth transition into home care and sets the foundation for positive patient outcomes.

Responsibilities

  • Create the initial comprehensive and interdisciplinary patient centered home health plan of care; and update the home health plan of care in collaboration with the physician or other authorized practitioner (AP).
  • Ensure agency compliance with regulatory and organizational requirements.
  • Demonstrate expert knowledge unique to the practice of community health nursing.
  • Provides high quality clinical care and teaching.

Qualifications

  • Associates Diploma in Nursing, required; Baccalaureate Degree, highly preferred.
  • Current RN License in NY (in good standing).
  • Minimum of one year of clinical experience in a Certified Home Health Agency.
  • Knowledge of Microsoft Office and experience documenting in a clinical software system preferred.
  • Driver’s License required in all boroughs except Manhattan.
  • Strong assessment skills, clinical judgment, and critical thinking abilities.
  • Ability to work independently in a community setting. Min USD $112,000.00/Yr. Max USD $134,100.00/Yr.

Requirements

Candidates must hold a current NY RN license and an Associate's degree in Nursing, though a Bachelor's degree is highly preferred. A minimum of one year of clinical experience in a Certified Home Health Agency and a valid driver's license are required.
