Social Worker - Full-Time - Onsite Must have experience working in a skilled nursing facility. We are seeking a compassionate, resident-focused Social Worker to join our Skilled Nursing Facility team. In this role, you will advocate for residents, support families, and help ensure the highest quality of care and quality of life for those we serve.
The Social Worker is responsible for assessing residents' psychosocial needs, coordinating services, facilitating care planning, and providing emotional support to residents and families throughout their stay. This position works collaboratively with the interdisciplinary team to promote resident well-being, successful transitions of care, and compliance with regulatory requirements.

Candidates must have a Bachelor's Degree in Social Work, Psychology, Gerontology, or Sociology and at least 2 years of related experience, including 1 year in a healthcare setting. Proficiency in MS Office and compliance with Florida’s Care Provider Background Screening are required.
