Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring an Intake Registered Nurse (RN) – Float for Element Care PACE in Lowell, Massachusetts.
POSITION OVERVIEW Complete comprehensive in-home nursing assessments for prospective PACE participants.
Evaluate medical, functional, psychosocial, and environmental needs to determine program eligibility.
Collaborate with the interdisciplinary team to develop individualized plans of care.
Serve as a float RN supporting multiple PACE centers throughout the service area.
Coordinate with physicians, therapists, social workers, and community partners to ensure seamless participant transitions.
Maintain accurate documentation utilizing the electronic medical record.
COMPENSATION & BENEFITS Competitive salary from $80,000-$103,000 based on experience.

MINIMUM REQUIREMENTS Active Registered Nurse (RN) license in Massachusetts. Minimum one year of geriatric nursing or adult mental health nursing experience. Ability to complete comprehensive in-home nursing assessments. Strong assessment, communication, and documentation skills. Comfortable working independently while collaborating within an interdisciplinary team. Electronic medical record experience. Reliable transportation for travel between participant homes and multiple PACE sites.
