Position overview
Join an interdisciplinary PACE team dedicated to helping older adults remain independent while living safely in their homes.
Deliver restorative and rehabilitative occupational therapy services under the supervision of a licensed Occupational Therapist.
Provide care in both the PACE center and participant homes with a focus on maximizing independence, safety, and activities of daily living (ADLs).
Collaborate closely with physicians, therapists, nurses, and the interdisciplinary team to implement individualized care plans.
