About the role

Provide comprehensive pharmaceutical care and medication management for diverse patients in an outpatient FQHC setting. Responsibilities include dispensing medications, conducting patient visits for chronic conditions, and supervising pharmacy technicians.

First Choice Health Centers is seeking a part-time Staff Pharmacist for our community health pharmacy located in East Hartford, CT. This position is accountable for providing critical, pharmaceutical care and guidance to our diverse patient population, while guiding and supporting our patients on their journey to better health and wellness.

*This is a 24 hour position; Monday, Tuesday and Friday hours are preferred with room for flexibility with shifts.

Experience and Training

  • Graduated from an accredited Doctor of Pharmacy (PharmD) or Bachelor’s Degree in Pharmacy program; residency preferred
  • 2-5 years of experience preferred; comfortable working in administrative capacity or working behind the “pharmacy bench”
  • A commitment to serving diverse and vulnerable populations with compassion and respect.
  • Excellent communication, interpersonal, and organizational skills.
  • Completion of an accredited Immunization Training Program
  • Active, unrestricted Pharmacy license to practice in the State of Connecticut
  • BLS Certification through the American Heart Association or American Red Cross.
  • Pay is based on several factors including but not limited to work experience certifications, etc. Range for this position is $60.00 to $68.00/hour.

Standard Job Duties

  • Provide comprehensive medication management for patients in an outpatient Federally Qualified Health Center (FQHC) setting, with a focus on chronic disease management, medication adherence, and preventive care
  • Conduct patient visits for chronic and complex conditions such as diabetes, hypertension, asthma/COPD, and anticoagulation therapy
  • Collaborate with primary care providers, behavioral health clinicians, and care teams to optimize medication therapy and improve patient outcomes
  • Review, interpret, and accurately dispense medications in accordance with Federal and State laws
  • Counsel patients and families on safe and effective medication use, including side effects, interactions, and contraindications
  • Support patient access to affordable medications through formulary review, prior authorizations, 340B program support, and insurance navigation
  • Maintain current knowledge of 340B rules and regulations, including accurate dispensing, auditing, and inventory control of 340B drugs
  • Provide immunizations, medication therapy management, and drug therapy reviews
  • Accurately process and bill prescription claims through insurance or cash-pay plans
  • Supervise pharmacy technician(s) in accordance with state requirements and company policy
  • Maintain pharmacy inventory, including stocking, labeling, storage, and expiration date monitoring
  • Ensure pharmacy operations comply with company policies, infection control standards, and state/federal regulations

Requirements

Requires a PharmD or Bachelor's in Pharmacy and an active Connecticut pharmacy license. Candidates should have BLS certification, immunization training, and preferably 2-5 years of experience.
