Pacific Coast Post-Acute is seeking a dedicated and compassionate Physical Therapist (PT) to join our exceptional in-house rehabilitation team in Salinas, California.

As a leading short-term rehabilitation and skilled nursing facility in Monterey County, we provide advanced sub-acute and restorative care to help residents regain mobility, independence, and quality of life. Key Responsibilities

Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on resident goals and functional needs.

Provide skilled physical therapy interventions, including gait training, therapeutic exercise, balance training, manual therapy, and neuromuscular re-education.

Monitor resident progress and adjust treatment plans to maximize functional outcomes.

Guide residents through their rehabilitation journey from acute recovery toward safe discharge and community reintegration.

Collaborate with interdisciplinary team members to coordinate effective, resident-centered care.

Supervise Physical Therapist Assistants (PTAs) in accordance with California practice guidelines.

Maintain accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.

Participate in care conferences, clinical meetings, and quality improvement initiatives.
