Position Summary

As a Physical Therapist (PT), you will evaluate patients, develop individualized Plans of Care, and provide skilled therapy services in patients' homes throughout the assigned service territory. You will lead the rehabilitation process by improving mobility, increasing strength, reducing fall risk, and helping patients regain the highest level of functional independence possible.

This role offers the opportunity to build meaningful relationships with patients while enjoying the autonomy, flexibility, and purpose that home health provides.

Responsibilities

  • Perform comprehensive physical therapy evaluations, Start of Care (SOC), Resumption of Care (ROC), Recertification, Reevaluation, and Discharge assessments.
  • Complete accurate and timely OASIS documentation in accordance with Medicare and agency standards.
  • Develop individualized Plans of Care with measurable functional goals and evidence-based interventions.
  • Provide skilled therapeutic exercise, gait training, balance activities, transfer training, fall prevention strategies, and functional mobility interventions.
  • Monitor patient progress and modify treatment plans based on clinical findings and patient goals.
  • Supervise Physical Therapist Assistants (PTAs) and ensure quality, compliant delivery of care.
  • Educate patients and caregivers on home exercise programs, safe mobility techniques, assistive devices, and strategies to maximize independence.
  • Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate exceptional patient care.
  • Complete timely, accurate clinical documentation that reflects skilled need, patient progress, and medical necessity.
  • Demonstrate accountability and ownership in achieving outstanding patient outcomes while maintaining compliance with all agency, state, and federal regulations.

Qualifications

  • Graduate of an accredited Physical Therapy program.
  • Current Ohio Physical Therapist (PT) license in good standing.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and automobile insurance.
  • Home health and OASIS experience preferred but not required.
  • Strong evaluation, clinical reasoning, documentation, communication, and organizational skills.
  • Passion for delivering exceptional care and helping patients achieve meaningful outcomes.

Requirements

Must be a graduate of an accredited Physical Therapy program with a current Ohio PT license and CPR certification. Requires a valid driver's license, reliable transportation, and strong clinical documentation skills.
