The Registered Nurse (Variable) provides comprehensive palliative nursing care through assessment, care planning, symptom management, patient and family education, care coordination, and interdisciplinary collaboration. This position serves as a case manager for an assigned patient caseload while ensuring compassionate, patient-centered care that supports quality of life for individuals living with serious illness. The RN collaborates with physicians, community partners, and the interdisciplinary team to coordinate care across multiple settings while promoting HopeWest's mission and values.The registered nurse is responsible for providing patient care to assigned patients within the life span of all ages.
must be available for at least 24 hours of work per month. They may identify the days and shifts they are available but must include 1 weekend shift. A general availability should be provided, with a more detailed availability submitted the month prior to scheduling. PRN's are expected to work up to the 24 hours per month, unless no shifts are available for the approved divisions they are trained or cleared for.

Candidates must hold an unrestricted Colorado RN license and a valid driver's license with auto insurance. A minimum of one year of experience is required, with a preference for those from an acute care setting.
