Evaluate patients to develop and implement individualized physical therapy treatment plans within a home health setting. Collaborate with an interdisciplinary team to improve patient mobility and independence while maintaining accurate Medicare documentation.
Evaluate patients and develop individualized physical therapy treatment plans. Provide skilled physical therapy services in patients' homes according to physician orders and the plan of care. Assess patient progress and modify treatment plans as clinically appropriate. Educate patients, families, and caregivers on therapeutic exercises, mobility, fall prevention, and home safety. Complete timely and accurate documentation in accordance with Medicare, CMS, and agency guidelines. Collaborate with physicians, nurses, occupational therapists, speech therapists, home health aides, and other members of the interdisciplinary team. Participate in care coordination and case conferences as needed. Maintain compliance with agency policies, state regulations, and accreditation standards. QualificationsQualifications: Current Illinois Physical Therapist license required. Minimum of one year of home health experience preferred. Experience with Medicare home health regulations and documentation preferred. OASIS experience preferred. Experience with Axxess EMR is a plus. Excellent communication, clinical assessment, and organizational skills. Current CPR certification. Valid driver's license, reliable transportation, and current auto insurance.
