Provide skilled physical therapy treatments and follow established plans of care to help patients improve mobility and independence in their homes. Document patient responses and collaborate with an interdisciplinary team of nurses and physicians.
Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist. Follow the established plan of care and treatment goals developed by the supervising Physical Therapist. Observe, document, and report patient responses to treatment and communicate changes in condition to the supervising Physical Therapist. Educate patients and caregivers on prescribed exercises, mobility techniques, fall prevention, and home safety. Complete accurate and timely clinical documentation in accordance with Medicare, CMS, and agency guidelines. Collaborate with nurses, therapists, physicians, and other members of the interdisciplinary care team. Participate in care coordination and case conferences as needed. Maintain compliance with agency policies, state regulations, and accreditation standards. QualificationsQualifications: Current Illinois Physical Therapist Assistant (PTA) license required. Minimum of one year of home health experience preferred. Knowledge of Medicare home health regulations and documentation preferred. Experience with Axxess EMR is a plus. Strong clinical, communication, and organizational skills. Current CPR certification. Valid driver's license, reliable transportation, and current auto insurance.
