About the role

Provide short-term, intensive home-based crisis intervention to stabilize families and prevent psychiatric hospitalizations for youth. Responsibilities include conducting intakes, creating individualized treatment plans, and collaborating with community providers to connect clients to resources.

Clarity Wellness Community is a progressive not-for-profit organization providing behavioral health services in Allegany and Wyoming counties. Clarity's mission is to empower people with care, connections and support for a lifetime of choice. We are currently seeking a full-time HBCI Provider to support children and families on their path to wellness.

Home-Based Crisis Intervention Specialist aim to empower and stabilize families by providing short-term (4-6 weeks), intensive support to avoid unnecessary psychiatric hospitalizations of children and youth ages 5-21 years. This is accomplished using evidence-based practices to assist youth and their families with safety planning, and obtaining needed medical, social, recreational, educational, financial, vocational, and other services through assessment, goal setting, progress monitoring, and collaboration with other professionals.

Responsibilities

  • Provide flexible family centered service by scheduling sessions at convenient times
  • Conduct timely intakes, assessments, and individualized treatment plans focusing on recovery and strength
  • Offer crisis intervention, de-escalation, safety planning, and motivational support to youth and families
  • Collaborate with families and community providers to communicate care, set goals, and connect clients to appropriate resources

Qualifications

A Master's degree in Psychology, Social Work, Mental Health or related field required. Applicants are expected to be licensed by the New York State Education Department and operate within the practitioner's scope of practice as defined in NYS law. Licensed mental health providers (LMHP's) include: Licensed Psychologists, Licensed Clinical/Masters Social Workers, Licensed Marriage and Family Therapists, Licensed Mental Health Counselors, and Licensed Creative Arts Therapist. Unlicensed MA staff with at least one year of relevant experience in community-based mental health or case management may be considered.
