Deliver community support services and case management to children and adults with disabilities in various community settings. Provide training in daily living skills and coordinate with treatment teams to monitor recovery goals.
Qualifying experience includes service delivery to individuals with mental illness, substance abuse disorders, or developmental disabilities. Must hold or obtain by start date, MO Class E Driver's License (Class D for IL Residents), and proof of continuous auto insurance. Must meet CCC's Motor Vehicle Record standards and have reliable transportation. Travel in own vehicle is required. The Program Community Psychiatric Rehabilitation Center (CPRC) supports adults and children with disabilities in social role functioning and daily living skills. Staff work closely with clients in various community settings, providing case management services and therapy to promote recovery.
The Job Responsible for delivering community support services for assigned clients within the CPRC. The role involves working in the community, such as client homes or residential centers. Typically, the position manages 20-25 clients, meeting with them weekly.
Essential Functions Provide community support services to children and adults enrolled in the CPRC program.
Maintain a caseload, including community and hospital settings.
Document client interactions per agency guidelines, completing all documentation by deadlines.
Provide training/education to clients in daily living skills.
Monitor treatment plan goals and maintain contact with the treatment team.
Refer clients to appropriate community resources to improve functionality and develop a support system.
Document according to clinical policies, DMH, CARF, and federal/state program standards.
