About the role

The LPN provides direct and indirect patient care, including medication administration and wound care, under the supervision of the House Supervisor. They are responsible for assessing patient conditions, maintaining accurate records, and collaborating with the healthcare team to implement care plans.

Description The Licensed Practical Nurse (LPN) in the Medical/Surgical department is responsible for providing direct and indirect patient care services, within the scope of practice, under the supervision of the House Supervisor and the direction of the Director. This role includes assessing, planning, implementing, and evaluating patient care in collaboration with the healthcare team. The LPN must adhere to the hospital’s policies and procedures, maintain accurate patient records, and ensure a safe and therapeutic environment for patients and their families.

Requirements

Essential Functions Provide direct patient care, including medication administration, assessments, wound care, and patient education. Collaborate with the healthcare team to develop, implement, and revise patient care plans as indicated by the patient’s response to treatment. Monitor and assess patient conditions, intervene as necessary, and report changes to the physician. Maintain accurate and up-to-date patient records, including nursing assessments, medication administration, education, care plans, and progress notes. Educate patients and their families about their medical condition, treatment, plan of care, and self-care techniques. Ensure a safe and therapeutic environment for patients, families, and staff. Adhere to infection control, safety, and regulatory policies and procedures. Participate in quality improvement initiatives and contribute to a culture of continuous learning and professional development. Provide support and guidance to nursing assistances and other staff members. Performs other duties and responsibilities as assigned.

Qualifications

Graduate from an accredited school of nursing and maintain current licensure as a Licensed Practical Nurse by the State of Missouri Maintain Basic Life Support (BLS) Previous experience in Medical/Surgical or similar setting preferred. Strong clinical assessment and critical thinking skills Excellent communication and interpersonal skills Ability to work effectively in a fast-paced environment. Commitment to providing high-quality, patient-centered care. Willingness to learn and adapt to new technologies and practices. IV Certification
