Make a Difference Every Day We are seeking a compassionate Registered Nurse (RN) to join our Assertive Community Treatment (ACT) team. In this role, you'll provide community-based psychiatric nursing services to adults living with serious mental illness while collaborating with an interdisciplinary team dedicated to promoting recovery and independence.
Coordinate medical care and annual health evaluations for ACT clients. Administer injectable psychotropic medications under psychiatric direction. Monitor lab results, vital signs, medications, and medical appointments. Coordinate with physicians, pharmacies, hospitals, and community providers. Conduct home visits as needed and respond to medical emergencies. Participate in treatment planning, clinical meetings, and maintain timely Medicaid Rule 132/140 documentation. Current Illinois Registered Nurse (RN) license. Minimum of 2 years' experience working with individuals with serious mental illness. Valid Illinois driver's license and ability to travel within the community. Ability to pass a background check and complete required annual training.
