Community Health Alliance is looking for an LPN to provide care in a Level 3.7 Withdrawal Management residential treatment facility. LPNs in our program work closely with RNs, Nurse Practitioners, Physicians, and Behavioral Health Technicians to deliver high-quality, trauma‑informed care in a structured residential environment. In this role, you will provide 24/7 medically monitored care to individuals experiencing acute withdrawal symptoms, ensuring safety, stabilization, and dignity throughout the detox process. We are hiring for

  • LPNs to support 24/7 operations on the following shifts: 1st shift LPN's starting pay is $30.00 per hour and 3rd shift LPN's will receive a $4.00 per hour shift differential. Duties and Responsibilities

Complete nursing tasks as directed by a registered nurse or medical provider. Provide medically monitored withdrawal management in accordance with ASAM 3.7 standards. Conduct nursing assessments, including vitals, withdrawal scales (e.g., CIWA/COWS), and ongoing symptom monitoring. Administer medications safely and accurately, including PRN withdrawal medications per protocol. Participates as part of an interdisciplinary team coordinating patient care. Monitors patient appointments and collects health information and vitals for medical provider. Educates patients on monitoring and managing symptoms, medication and health conditions. Collects blood, urine and other specimens as needed for laboratory testing. Administers injections, TB testing, vaccinations as ordered by medical provider. Completes health and symptomology screenings to aid in patient care. Coordinates with other medical facilities to obtain medical records for treatment of patients. Implements appropriate nursing interventions and actions according to established plan of care within scope of practice. Support clients through the withdrawal process with empathy, professionalism, and therapeutic communication. Document all care in the electronic health record (EHR) in compliance with state and federal regulations. Practice within the scope of practice as outlined by the Ohio Board of Nursing. Qualifications Licensed in the State of Ohio as a Licensed Practical Nurse (LPN). Two years related experience in withdrawal management, behavioral health, residential treatment, or acute care preferred. Current BLS/CPR certification required. Valid driver's license and reliable transportation (travel between sites required). Ability to work flexible hours, including early morning, weekends or evenings if needed. Experience in substance use treatment, behavioral health or related nursing field preferred. Knowledge of medication-assisted treatment (MAT), harm reduction and trauma-informed care is preferred. Commitment to providing nonjudgmental, empathetic care to individuals with substance use and behavioral health disorders. Effective verbal and written communication skills. Proficient with Microsoft 365 applications, including Office Suite, Outlook and Teams. Adept at learning and navigating electronic health record systems. Requirements

Must be a Licensed Practical Nurse (LPN) in Ohio with current BLS/CPR certification and a valid driver's license. Preference is given to candidates with two years of experience in behavioral health or withdrawal management.
