The Outpatient Therapist plays a vital role in our mission to deliver coordinated, individualized care to our communities. As an Outpatient Therapist , you will provide: Case Management Services: Coordinate and manage services for clients, ensuring they have access to necessary resources and support. Service Plans: Develop individualized, written service plans to address clients’ specific needs and goals. Crisis Intervention: Provide immediate support and intervention during mental health crises. Evaluations and Assessments: Conduct psychological evaluations and write diagnostic assessments, including those for behavioral, substance abuse, and economic self-sufficiency issues. Facilitate Wraparound Meetings: Organize and lead team meetings to ensure coordinated care among providers and natural supports. Master’s degree in Counseling, Social Work, Psychology, or a related field.
