About the Role

The Licensed Practical Nurse (LPN) in this part-time day shift position will provide essential nursing care within a hospital-based Skilled Nursing Facility (SNF).

The primary goal is to deliver high-quality, compassionate care to patients recovering from acute illness or surgery, ensuring their safety and comfort throughout their stay.

This role involves collaborating closely with registered nurses, physicians, and other healthcare professionals to implement individualized care plans and monitor patient progress.

The LPN will also play a critical role in patient education, assisting with medication administration, and documenting clinical information accurately.

Ultimately, this position supports the facility’s mission to promote patient recovery and improve overall health outcomes in a dynamic healthcare environment.

Minimum Qualifications

  • Current and valid Licensed Practical Nurse (LPN) license in the state of employment.
  • Completion of an accredited practical nursing program.
  • Basic Life Support (BLS) certification.
  • Demonstrated knowledge of nursing principles, patient care techniques, and medication administration.
  • Ability to communicate effectively with patients, families, and healthcare team members.

Preferred Qualifications

  • Experience working in a Skilled Nursing Facility or hospital-based setting.
  • Certification in geriatric nursing or wound care.
  • Familiarity with electronic health record (EHR) systems.
  • Additional certifications such as IV therapy or phlebotomy.
  • Strong organizational skills and ability to manage multiple priorities in a fast-paced environment.

Responsibilities

  • Administer medications and treatments as prescribed by physicians and according to facility protocols.
  • Monitor and record patients’ vital signs, symptoms, and overall condition, reporting any changes promptly to the healthcare team.
  • Assist patients with activities of daily living, including hygiene, mobility, and nutrition, while promoting independence whenever possible.
  • Collaborate with interdisciplinary team members to develop, implement, and update individualized care plans.
  • Maintain accurate and timely documentation of nursing care, patient responses, and any incidents in compliance with regulatory standards.
  • Provide emotional support and education to patients and their families regarding health conditions and care procedures.
  • Ensure a safe and clean environment by adhering to infection control policies and safety regulations.

  • The required skills enable the LPN to provide safe and effective patient care by accurately administering medications, monitoring patient status, and documenting clinical information.

Requirements
