Description Klarus Hospice is seeking compassionate and dedicated PRN / Per Diem Registered Nurses (RN) to join our team. We are looking for RNs who are passionate about providing quality hospice care to our community-based patients in San Antonio, TX and the surrounding areas.

Key Responsibilities: Follow a designated care plan in accordance with patient needs. Make home visits to clients in designated geographic territories. Perform assigned duties, including medication administration, wound care, treatments, and procedures. Monitor client conditions and report changes to the Clinical or Client Services Manager. Follow up with, execute, and properly document doctors' orders. Perform client assessments. Provide case management and coordination of care. Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet. Job Qualifications: Current State or Compact Registered Nurse (RN) License. A minimum of one year of clinical experience is preferred. Oasis knowledge and /or end of life care experience preferred. Demonstrate knowledge and skillset in current nursing practice. Possess a valid state driver's license and automobile liability insurance. Knowledge of HCHB EMR is desired.
