About the Role

The CNA Hospice Home Team member plays a vital role in providing compassionate, patient-centered care to individuals receiving end-of-life services wherever they call home.

This position focuses on supporting patients' comfort, dignity, and quality of life by assisting with daily living activities and monitoring their physical and emotional needs.

The role requires close collaboration with interdisciplinary hospice teams, including nurses, social workers, and chaplains, to ensure holistic care tailored to each patient’s unique situation.

The CNA will also provide essential support and education to family members and caregivers, helping them navigate the challenges of hospice care.

Ultimately, this position contributes significantly to the mission of delivering respectful, empathetic, and professional hospice care in a home setting across the United States.

Minimum Qualifications

  • Current and valid Certified Nursing Assistant (CNA) certification in the state of practice.
  • High school diploma or equivalent.
  • Previous experience in a healthcare setting, preferably in hospice, home health, or long-term care.
  • Ability to pass background checks and meet health screening requirements.
  • Strong communication skills and the ability to work effectively as part of a multidisciplinary team.
  • Ability to travel within our coverage territory: Manitowoc, Sheboygan, & Ozaukee County
  • Ability to work 1st shift hours Monday - Friday with a rotating weekend requirement

Responsibilities

  • Assist patients with activities of daily living such as bathing, dressing, grooming, toileting, and mobility to maintain their comfort and dignity.
  • Monitor and document patients’ physical and emotional conditions, reporting any changes promptly to the nursing staff.
  • Provide emotional support and companionship to patients and their families, fostering a compassionate and reassuring environment.
  • Collaborate with the hospice interdisciplinary team to implement individualized care plans and ensure continuity of care.
  • Educate family members and caregivers on patient care techniques, safety measures, and symptom management as directed by clinical staff.
  • Maintain accurate and timely documentation of care provided, adhering to regulatory and organizational standards.
  • Adhere to infection control protocols and safety guidelines to protect patients, families, and staff.
