The Ambulatory Care Manager utilizes the nursing process to help members/ families that have experienced a critical event or have a diagnosis requiring extensive resources. Ensure care is delivered in an efficient and cost-effective manner in an effort to achieve optimal clinical outcomes.
This is a part time (non-benefitted), grant funded position. The expectation is that this person will work 8-20 hours per week.
Minimum $52.93 Midpoint $70.13 Maximum $87.32
Rate of pay is determined by various factors, including but not limited to knowledge, skills, competencies, experience, education, as well as position requirements.
