Conduct comprehensive psychosocial assessments for hospice patients. Collaborate with the interdisciplinary team to develop individualized care plans. Deliver services in private homes, Adult Family Homes, Assisted Living, Independent Living, and Skilled Nursing Facilities. Qualifications Master’s Degree in Social Work (MSW), Marriage and Family Therapy (MFT), or Mental Health Counseling (MHC) from an accredited institution. Minimum of 2 years of social work experience (1 year in Hospice or Home Health preferred). Strong verbal and written communication skills. Proficiency in electronic charting and documentation. Valid driver’s license, auto insurance, and reliable vehicle required.
