What you'll do
Perform comprehensive physical therapy evaluations and establish individualized treatment plans.
Develop and implement therapeutic interventions to improve mobility, strength, balance, and functional independence.
Monitor patient progress, modify treatment plans as needed, and document outcomes.
Educate patients, families, and caregivers on exercises, safety, and disease management.
Collaborate with physicians, nurses, and the interdisciplinary care team to ensure coordinated patient care.
Maintain timely, accurate, and compliant clinical documentation.
Follow infection control practices and all agency policies and procedures.
Promote exceptional patient care while supporting the mission and values of the agency.

Must be a graduate of an accredited Physical Therapy program with a current state license and CPR certification. A valid driver's license and reliable transportation are required, with home health experience preferred.
