Responsibilities

Coordinates and oversees all aspects of new patient admissions Collaborates with patient care teams to ensure seamless communication Educates families on patient condition, needs, and care Performs head-to-toe physical assessments Timely and accurately completes documentation Other duties as assigned

Compensation

This role provides a competitive base salary. Our starting range is between $85,000 to $95,000 annually. Pay is dependent on applicant's relevant experience.

Requirements

Licensure: Current State license as a Registered Nurse. Current driver’s license. Current CPR Certification.

Experience

Two (2) years' experience as a Registered Nurse in a clinical care setting and one (1) year full-time hospice preferred.

Skills

Nursing skills as defined as generally accepted standards of practice. Knowledge of state and federal regulations. Ability to work independently and make sound decisions. Strong attention to detail and organizational habits. Excellent verbal and written communication skills. Good interpersonal skills.

Transportation

Reliable transportation and valid auto liability insurance.
