The Peer Support Specialist provides integrated, whole-person care by performing outreach and coordinating services for members. They collaborate with an interdisciplinary team to address physical and behavioral health needs while connecting members to community resources.

Blue Ridge Health is currently seeking a Peer Support Specialist to be part of our Tailored Care Management team.

Looking for a Peer Support Specialist to support Rutherford and Polk County, NC.

What You'll Do

The Peer Support Specialist plays a unique and vital role in supporting one’s journey of recovery. They serve on an interdisciplinary tailored care management team whose focus is to deliver integrated, whole person care addressing a member’s physical health, behavioral health, long-term services and health needs and/or pharmacy needs. Responsibilities include:

Performing general outreach, engagement, and follow-up with members

Coordinating services/appointments (e.g., appointment/wellness reminders, arranging transportation)

Engaging in health promotion activities (as defined in the Tailored Care Management Provider Manual)

Sharing information with the care manager and other members of the care team on the member’s circumstances

Providing and tracking referrals and providing information and assistance in obtaining and maintaining community-based resources and social support services.

Participating in case conferences.

Support the care manager in assessing and addressing unmet health-related resource needs.

Disclaimer

This role requires flexibility to transport clients/patients to organization-sponsored services or resources that address their basic and clinical needs. Drivers transporting clients/patients or driving company vehicles are to be age 21 or greater. For those using their personal vehicles to transport clients/patients, a certificate of insurance is required demonstrating at least the minimum required NC auto liability insurance.

What We're Looking For

  • High school diploma or equivalent required
  • Certified Peer Support Specialist with lived behavioral health experience.
  • Valid Driver’s License
  • Previous Peer Support Specialist, Community Health Worker, Community Navigators or Family Navigators a plus

Requirements
