About the role

Perform in-person and virtual clinical eligibility assessments and utilization reviews for various long-term care programs. Support older Vermonters and people with disabilities in receiving services in their preferred settings.

As part of the Department of Disabilities, Aging and Independent Living (DAIL) Adult Services Division, the Choices for Care program helps older Vermonters and people with disabilities receive long term care services and supports in the setting of their choice.

The Nurse Case Manager/Utilization Review Nurse role is to perform in-person/virtual clinical eligibility assessments and utilization review for Vermont's Long-term Care Medicaid Choices for Care program, Brian Injury program, Attendant Services program and for the Adult High Technology Program.

This position is based in Burlington and will include regular travel to neighboring counties.

Work is performed under the general supervision of the Nurse Administrators and program manager.

RN licensure is required, experience with case management preferred.Who May Apply This position, Nurse Case Manager / URN I - Temporary (Job Requisition #55130), is open to all State employees and external applicants.

This is a temporary position.

Temporary work is typically for variable hours, and is not expected to be full-time.

The State does not guarantee 40 hours of work per week.

Please note that multiple positions in the same work location may be filled from this job posting.Minimum Qualifications Possession of or eligible for licensure as a Registered Nurse RN in Vermont OR eligible to practice in the state of Vermont via a multi-state license AND three 3 years or more of professional nursing experience in an acute hospital setting, long term care, health insurance carrier, or within a community health/public health setting.

Preferred

Bachelor of Science in Nursing BSN.

NOTE

Must maintain Vermont licensure as a Registered Nurse OR eligible to practice in the state of Vermont via a multi-state license as a condition of employment.

Candidates must pass any level of background investigation applicable to the position. Requirements

Requires a valid Registered Nurse (RN) license in Vermont or a multi-state license and at least three years of professional nursing experience. A Bachelor of Science in Nursing (BSN) and reliable personal transportation are preferred.
