Assist participants in achieving goals outlined in their Individualized Service Plans (ISPs). Provide guidance and encouragement in developing communication, social, self-advocacy, and independent living skills. Accompany participants on community outings and facilitate participation in recreational, volunteer, educational, and vocational activities. Safely transport participants to and from the program site and community activities using agency vehicles or, when necessary, a personal vehicle in accordance with agency policy. Provide personal care assistance, including mobility, hygiene, and activities of daily living, as needed. Monitor participant health, behavior, and safety, reporting concerns or incidents promptly to the Program Team Lead. Complete accurate and timely documentation, including attendance records, daily progress notes, and incident reports. Collaborate with the Program Team Lead and interdisciplinary team members to provide consistent, high-quality services. Maintain professional communication with participants, families, guardians, and community partners. Participate in staff meetings, required trainings, and ongoing professional development. Perform other duties as assigned to support program operations.
  • QualificationsRequired
  • Qualifications: High school diploma or equivalent. Prior experience supporting individuals with developmental or intellectual disabilities or in a similar human services role. Strong interpersonal skills and the ability to build positive, respectful relationships. Ability to follow individualized plans and provide person-centered support. Ability to complete and maintain CPR/FA and SafetyCare certifications. Valid driver’s license and reliable transportation. Working Environment This position is community-based and requires travel to homes and community settings. Candidates must be comfortable working in a variety of environments and utilizing technology for documentation and communication. Ready to Make an Impact? Requirements

A high school diploma or equivalent and a valid driver's license with reliable transportation are required. Candidates should be able to obtain CPR/First Aid and SafetyCare certifications and be physically capable of lifting up to 25 pounds.
