As a Case Manager supporting our Individual Home Supports (IHS) and Community-Based Day Services (CBDS) programs, you'll play a vital role in helping individuals achieve personal goals, build relationships, access community resources, and strengthen independent living skills. This flexible, hybrid position floats between our CBDS and Individual Home Supports programs, providing both direct support and case management services based on individual and program needs. You'll work closely with individuals, families, community providers, and interdisciplinary teams to promote choice, inclusion, and personal growth. This position is Relief / As Needed and does not guarantee a set schedule or number of hours. ASL fluency is strongly preferred. - Sunday-Saturday, 9:00 AM – 6:00 PM Location Sandwich, MA Community-based position with regular travel throughout the service area What You'll Do Provide direct support to individuals in home, community, and program-based settings Deliver individual supports case management, including coordination of services and community resources Promote individual choice, self-expression, independence, and community inclusion Facilitate assessment, implementation, and documentation of individual goals and treatment objectives Provide skill-building support, supportive counseling, and crisis intervention as outlined in service plans Coordinate access to medical providers, community services, and natural supports Assist individuals in developing and maintaining meaningful relationships Provide transportation to appointments, activities, and community-based programming as needed Maintain accurate and timely documentation in compliance with KDC and funding agency requirements Collaborate with interdisciplinary teams, medical professionals, and community partners Participate in staff meetings, supervision, and ongoing training opportunities Ensure all individuals are treated with dignity, respect, and confidentiality
- QualificationsRequired
- Qualifications: Minimum of 1 years’ experience in case management, direct support, or a related human services role. Knowledge of person-centered planning, developmental disabilities, and community resources. Strong organizational, documentation, and communication skills. Ability to work independently while maintaining collaborative relationships with teams, caregivers, and community partners. CPR/First Aid Certified (or ability to obtain certification within designated timeframe). Valid driver’s license, reliable transportation, and willingness to travel extensively.
Preferred Qualifications
Experience in Shared Living, Adult Foster Care, or home-based support models. Experience supporting individuals living independently or in community-based settings. Familiarity with Medicaid, MassHealth, and related funding sources. Knowledge of Electronic Health Record (EHR) systems. Bilingual or multilingual abilities.
