The Case Manager works closely with a multidisciplinary treatment team—including clinical staff, admissions, nursing, and business development—to facilitate patient recovery. This role ensures patients have thorough aftercare plans, addresses individual needs, and supports patients in achieving the best possible outcomes.
Key Responsibilities
Complete patient admission assessments and document in Avatar within 72 hours of admission.
Collaborate with patients and support systems to create and implement individual Continued Care Plans (CCP).
Address patient needs across life domains: housing, employment, legal, financial, family, health, relapse prevention, and more.
Schedule post-discharge appointments, including SUD/MAT and primary care follow-ups.
Facilitate weekly Aftercare/Next Steps group meetings and Guesting sessions for new patients.
Participate in Multidisciplinary Team (MDT) meetings and contribute to discharge planning discussions.
Conduct outreach calls to patients who leave treatment early to re-engage them with recovery resources.
Document patient progress, referrals, and discharge planning in compliance with HIPAA and organizational standards.
Ensure effective communication with post-discharge providers and other relevant parties.
Skills & Qualifications
• Bachelor’s Degree in social work, psychology, or a related field (or equivalent combination of education and experience)
**A former patient may not be hired at the same location where they were treated until a minimum of one year has elapsed since the completion of their treatment at that location. Former patients may be considered for employment at locations within RCA other than where they were treated within a year after treatment.
