Provide skilled one-on-one nursing care in the patient's home Perform tracheostomy (trach) care and suctioning as needed Manage G-tube feedings and care Administer medications as ordered Monitor and document patient condition and nursing interventions Communicate effectively with the patient's family and healthcare team Ensure a safe, comfortable, and supportive care environment
Current Ohio Licensed Practical Nurse (LPN) license in good standing Experience with tracheostomy (trach) care required Experience with G-tube care and enteral feedings required Current CPR certification Excellent assessment, communication, and documentation skills Dependable, compassionate, and committed to providing exceptional patient care Pay & Benefits
