Responsible for case management and coordination of care for clients and families in residential treatment. Duties include facilitating educational groups, managing admissions, and coordinating aftercare and external provider referrals.

Case Coordinators are responsible for case management and coordination of care for clients/families in residential treatment including individual case management/family case management services to support client and family engagement in treatment, group facilitation for education and skill development in collaboration with the clinical team, and admission, aftercare/ discharge planning, and referral and external provider coordination and support.
Monday-Friday 8:00am - 4:00pm
$15 - $16/hr
