JOB SUMMARY The Care Manager performs professional assessments of an individual’s capabilities and the family’s capacity to provide care. They are responsible for coordinating and monitoring, client status, needs, changes, reassessments and complaints.

Duties and Responsibilities

Client contact must be made within three days of referral. Perform individual needs assessments and develop individualized service plans in a face-to-face interview. Service planning shall be completed within five working days of the assessment. Refer clients to the appropriate community/agency service providers. Coordinate informal and formal resources within five working days of the assessment. Confirmation of the arranged services are communicated to the providers and the referral source, with the appropriate documentation of such. Be responsible for monitoring services that are provided for the client and ensure that adequate services are received. Be responsible for maintaining case records on each client. Establish a working relationship with and an understanding of services provided by organizations/agencies in the counties. Communicate client needs with the In Home Services RN in the development of the In-Home Services Program. Assist with public relations/public awareness programs in area of service to inform the public of the availability of services. Be an advocate for the rights and needs of the elderly population. Be responsible for identifying and documenting service gaps in our region. Observe code of ethics and rules of confidentiality. Be responsible for assisting with In Home Services Program as needed and requested by management. This includes, but is not limited to, answering the telephone during billing time and assisting the Office Coordinator during RN’s absence. Maintaining the agency required 12 hours of in service, specific to the position. Perform other duties as assigned by supervisor and management. KNOWLEDGE, SKILLS, AND ABILITIES Knowledge of social work principles. Knowledge of various program eligibility criteria. Knowledge of health and human services, community, and state resources, including the application process. Ability to interview, obtain, and provide information relating to social and health problems and resources. Ability to work with others, especially older or disabled individuals. Ability to provide guidance and support to clients. Ability to record complete and accurate case notes. Ability to communicate effectively, both orally and in writing. Must possess excellent computer skills, including proficiency in Microsoft Office. Must provide own transportation, valid driver’s license and automobile liability insurance. Provide Supervisor with updated copies yearly as appropriate. Must be able to lift 40 lbs. Must have good hearing and vision. Must have dexterous use of both hands.

Minimum Qualifications

Must reside in or near the area of responsibility: and be licensed in the state of Arkansas as a social worker (Licensed Master Social Worker or Licensed Social Worker), a Registered Nurse or a licensed practical nurse; or have a Bachelor’s degree from an accredited institution in a health and human services field, plus two years’ experience in the delivery of human services to the elderly; or have performed satisfactorily as a case manager serving the targeted population for a period of two years(experience must be within the last three years). Requirements

Candidates must be a licensed social worker, RN, LPN, or hold a bachelor's degree in health and human services with two years of relevant experience. A valid driver's license, automobile insurance, and the ability to lift 40 lbs are required.
