  • PURPOSE: Social Worker assists residents and their families with emotional and social needs and ensures that needs are met and maintained on an individual basis. The Social Worker integrates social services with other elements of the care plan to maintain the highest feasible physical, mental, psychosocial well-being of each resident.

  • POSITION: Confers with resident and family members to develop a comprehensive social history and psychosocial assessment including the resident's concerns and cultural, religious and ethnic background preferences. Orients residents and their families to the facility, its services, limitations, and resident rights to ensure a smooth transition during admission to the facility. Completes sub-sections of the MDS assessment and accurately enters into the computer system, conferring with a nurse as needed. Completes the Pre-admission screening and resident review (PASRR) for specialized services for Medicaid to determine eligibility. Meets all submission timelines of reports/records/documentation as required by state and federal rules, regulations, and laws for healthcare facilities. Assists with the development, implementation and evaluation of the care plans for each resident in accordance with other health care providers and physicians. Empowers and encourage residents to be proactive in their care plans and decisions effecting them and being receptive and sensitive to patient requests. Provides support for residents and family members exhibiting emotional distress. Participates in quarterly care plan conferences for each resident, including family, physician, and care plan team. Actively participates in QAPI (Quality Assurance Process Improvement) program, taking a proactive approach to quality care and services on a continuous basis with engagement of residents, family members and staff. Demonstrates high standards of excellence for meeting resident needs. Communicates effectively, establishes rapport and maintains relationships with residents. Meets requirement of regular attendance to perform job duties. Has fun in creating a positive atmosphere for resident. Follows instructions and completes daily assignments as assigned and is proactive to change. Possesses understanding and compassion for the elderly population. Performs other duties as assigned.

QualificationsEDUCATION/REQUIREMENTS Bachelor’s Degree in Social Work, Human Services, or related field (required). One year of supervised social work experience in a health care setting, working directly with individuals (preferred) License/Certifications Iowa Board of Social Work licensed (preferred). CPR Certification (preferred). Required Skills and Abilities Resilience: ability to welcome constructive feedback, course correct and not take it personally Personability; ability to get along with all personality types and inspire trust with residents, staff, family. Ability to actively listen, with the goal of understanding. Ability to clearly speak and proficiently read, write and understand English.

Requirements

A Bachelor's degree in Social Work or Human Services is required, with a preference for a license from the Iowa Board of Social Work. Preferred qualifications include one year of supervised healthcare social work experience and CPR certification.
