Position Summary

Provides case management services to eligible consumers as defined by DPW Title 55 Code Chapter 5240. CCR’s mission is connecting people to services. And that’s exactly what you’ll be doing in this role. Here, you’ll appreciate the support and camaraderie of a local team as you provide the community with care and support that will improve their mental health and well-being. Essential Functions: The Blended Case Manager provides case management services to adolescents, children, and adults who suffer from a serious mental illness or emotional disturbance. An assigned blended case manager works with the client and families to access, link, coordinate and monitor needed services as well as to provide support and advocacy to assist clients in maintaining in their home or community. Complete all reports and documentation in a timely fashion. Maintain quality standards. Participates in an On-Call Rotation to ensure 24-hour coverage for holidays, weekends and daily, between the hours of 4:30 pm-8:30 am. Able to manage time effectively to meet the needs of consumers and program requirements that includes monthly utilization and on time completion of regulated documentation. Be well organized, have excellent verbal and written communication skills. Computer competency with the ability to learn agency programs. Other duties as assigned to achieve the mission of the agency. BCMs will be available to meet with consumers during normal business hours, as well as in the evenings and on weekends when scheduled, to meet the needs of the individual or when an individual is experiencing a crisis event. An on-call BCM will also be available via the on-call hotline 24 hours a day, 7 days a week. Mileage reimbursement is available at the state set rate! The starting rate is $21.54/hour but may start higher depending on experience and education. The On-Call hourly rate is $21.54/hr. and we pay a stipend of $250.00 per week when covering on call for one full week.

Qualifications

  • Bachelor’s degree in Human Services or related field. One year experience in MH Case Management or in the mental health field. Knowledge of case management principles, computer literate, familiarity with the Recovery Model, and knowledge of Clinton and Lycoming County resources. Valid driver’s license, proof of auto insurance, Act 33/34 and FBI clearances will be required. Individuals who have accessed human services are encouraged to apply. Essential Functions
  • Provide blended case management services to individuals in the community who qualify for services based on a mental health diagnosis. Complete all reports and documentation in a timely fashion. Maintain quality standards. Participates in an On-Call Rotation to ensure 24-hour coverage for holidays, weekends and daily, between the hours of 4:30 pm-8:30 am. Other duties as assigned. Physical Requirements: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Sufficient mobility to work in a normal office setting and use standard office equipment including a computer and navigation in, out and through various structures which may not be handicapped accessible. Vision to read printed materials and VDT screen. Hearing and speech sufficient to communicate in person or over the phone. Lifting to 20 lbs. maximum and occasionally carrying such articles as a laptop computer, files, ledgers, and small tools. Walking, standing and stair climbing will be necessary in carrying out duties.

Requirements

Requires a Bachelor's degree in Human Services or a related field and one year of experience in mental health case management. Candidates must possess a valid driver's license, auto insurance, and required FBI and Act 33/34 clearances.
