About the role

The Prescriber provides psychiatric assessments and medication management for patients in an outpatient Youth Psychiatry setting. Responsibilities include conducting evaluations, prescribing medications via office and home visits, and educating families on symptoms and side effects.

The Prescriber offers psychiatric assessment and medication management within the outpatient Youth Psychiatry setting in the Health Center Program.

This position is responsible for individual services to patients assigned by Crossroads Health.

This licensed provider will engage as a member of a collaborative team.

With a Trauma Informed approach, this position will be a representative for the organization and how we offer integrated services for recovery and mental health.

Additionally, the Prescriber will

Provide psychiatric assessments, medication evaluation and medication management. Regularly assess consumers & prescribe medications for clients via office and home visits Educate consumers & their families regarding medications/symptoms/illness/side effects. . Participate in ongoing assessment and therapeutic engagement. Complete, as directed, elements of the comprehensive assessment for clients within 60 days of admission.

Position

Requirements: Master's Degree (minimum) Licensed in the State of Ohio as a Physician Assistant, Psychiatric Nurse Practitioner, or Physician Controlled Substance Registration Certificate (DEA registration fees and licensure paid by agency) Medicare Provider Number Professional Liability Insurance. (employer provided) Standard Care Arrangement (annually) or Supervision Agreement (whichever applicable) (employer provided) BLS/First Aid (employer provided) Must have valid driver's license Our organization is committed to equal employment opportunity. Requirements

Candidates must hold at least a Master's degree and be licensed in Ohio as a Physician Assistant or Psychiatric Nurse Practitioner. A valid driver's license and DEA registration are also required.
