About the role

Provide therapeutic services for adults at an Adult Day Center to improve independence and collaborate with interdisciplinary staff. In the acute hospital setting, focus on early mobilization, post-surgical education, and safe discharge planning.

Due to growth and expanding therapy needs, we seek an exceptional State Board Licensed Physical Therapist to join our team in Woodland, California.

This hybrid position in Woodland, offers a flexible, part-time role split between the Yolo Adult Day Care Center and Woodland Hospital.

The Rehab team is solid and ready to welcome you.

The schedule can be 3-4 days per week, to equal approximately 16 hours per week at the Adult Day with a day or two at the hospital for a total of 20-30 hours per week, which, at 30 hours per week, would entitle you to a full benefit package.

The split can be a 50/50, Adult Day/Acute.

You will provide therapeutic services for adults at the Adult Day Center to improve their independence, working alongside nursing staff, activity coordinators, and Speech and Occupational Therapists.

In the acute hospital setting, a dynamic, collaborative environment, you will focus on early mobilization, educating patients post-surgical interventions and providing guidance on safe discharges.

Woodland Home Health/Acute maintains a good strong interdisciplinary team directed by an experienced, supportive and approachable Rehabilitation Coordinator.

Primary Responsibilities

  • Assessment, treatment planning and therapeutic interventions
  • Aid and direct patients in active and passive exercises, muscle re-education, activities of daily living, prosthetic training, gait and functional training
  • Educate patients and family on proper care plans
  • Communicate with team members to ensure quality care is being provided

Requirements

Candidates must be a State Board Licensed Physical Therapist. The role requires the ability to perform assessments, implement treatment plans, and educate patients and their families.
